2026-27 Required Student Health Documentation

Dear Pearl River Parents & Guardians,

In accordance with New York State regulations, student physical examinations are required  for all students in grades K, 1, 3, 5, 7, 9 and 11 and also for all students new to the Pearl River School District. In addition to the required grades, we recommend an annual physical for all students.

Physical examinations must be done within 12 months of school entry in order to comply with State requirements for the upcoming school year. Please note that we are only able to accept physicals on the required New York State form. Forms must be accompanied by an updated immunization record. The 2026-27 NYS immunization requirements have not changed from the 2025-26 school year. 

A physical dated on or after September 2, 2025 will fulfill NYS requirements.

Students who are unable to obtain a physical examination from their primary health provider should contact their school nurse. Students who are under-immunized and/or do not have appropriate immunization documentation on file in the nurse’s office will not be able to continue to attend school.

Required physical examinations and immunizations may be submitted at any time and must be submitted by September 1, 2026. The school nurses are accepting physicals and immunizations via email, fax or drop off at the building. 

 Additionally, you may submit an annual dental examination for your child.
